JCSD COVID-19 SYMPTOM SCREENING TOOL FOR STUDENTS
*Use this daily before school. Please contact the School Nurse at any time with questions you may have about the screening tool or illness symptoms.
Please take the student's temperature. If it is 100°F or higher, please keep the student home for 3 days, contact your primary care provider and your school nurse.
Question 1:
In the past 14 days, has the student been exposed to anyone diagnosed with COVID-19?
Question 2:
In the past 24 hours, has the student taken any medication to treat or reduce a fever, such as ibuprofen (i.e. Advil, Motrin) or acetaminophen (Tylenol)?
Question 3:
In the past 3 days, has the student experienced:
any ONE (or more) of the following symptoms in Group A,
OR
any TWO (or more) of the following symptoms in Group B (below)?
Group A | Fever (100°F or higher) |

Group B | Sore throat |
***If the student answered yes to ANY of the above 3 questions, keep student at home and contact your primary care provider before returning to school. Please notify the school nurse of any positive COVID-19 test or exposure.
*If you are tested for COVID-19, please stay home from school until given test results.
